Clients that plan to continue treatment with me are required to complete a medical records release form, preferably before or during the initial appointment. Please call for an appointment at the number provided below or you can send a message through psychology today or my website. We will need the following:

• Best phone number to reach you
• Current address
• Scheduled appt
• Current pharmacy

The phones/voicemail will be checked in the morning and at end of the day. Due to unavailability of medication records, pharmacy info, and demographics, please leave extensive detailed information if calling for medication issues and/or refills. For urgent and/or emergent matters, please go to nearest emergency room.
